A little sister would call her older brother 'Onii-chan', while a outsider could call him 'onii-san' without problems in my sight, calling someone with '-chan' is as well a childish way, but you can use with a girl you are close with or you are a senior.
eh, '-chan' being childish, I would say depends on how it's said more than anything.

Since from what I have seen, it is sometimes used between really close childhood friends (i.e. they view each-other as siblings).

As for those actually related using '-san' from what i know would just be disrespectful. (for siblings that is, for mother it is used... and I can't tell you why).

Cant see two grow up man calling each other with '-chan', normally they would just drop the honorifics anyway if they are that close, but well i still see it of them they being playful with each other.

Well, i get your vision that '-san' by siblings would be disrespectiful, since it's a polite and distant, but respectiful anyway.
I was just saying from the beginning that from us is okay we call him 'onii-san', since we're not close to him (neither i want to), but when i said onii-san it was because i was seing this word a bit too much, and mixed it here.

They didnt gave a name to him though now that i see, dont them? All drama and it's just 'onii-chan'
