The sister is implied to be pregnant
The sister is also being haunted by the death of her brother. Either she murdered him and is supressing it or she somehow feels that his death is her fault.

You missed the GF's brother,while pretending to be the GF, rapes the MC while he's asleep in the first chapter, and the MC got over it, but the MC couldn't get over his dad cheating on his mom.

I don't think there's enough time to pull this gotcha (or a reason to?) but just a thought - anorexia can cause a period to stop (one of the diagnostic criteria for anorexia used to be a menstruating woman not getting her period for 3+ months, though that was dropped). Stress can also affect periods, and it arguably could make you feel sick.

Rikimaru reacts poorly when touching women because of the trauma from the latter, but doesn't react beyond "huh, weird dream" to the former till chapter 11. In chapter 11 he goes acknowledges it as being real despite not being sure it was real or with Kagami up to that point. Maybe it was going to be a point of drama if this was continuing longer but it really sticks out that the flash of the idea that his girlfiend might have snuck in while he was asleep and sucked his dick doesn't make him respond more poorly than touching her hand.

this chapter reeks of another plot twist crammed in next chapter. like "oh actually it was kagami who fell and kyou watched her fall (as in her nightmare) and this chapter was actually kyou being delusional or a dream!!"; several things actually may hint at this stupid plot twist i came up with: "kyou's" ghost calling the hairclip that is objectively kagami's "mine", rikimaru suddenly losing his trauma, no great aunt visit, the nightmare that shows kagami's corpse on the ground as kyou watches. only unexplained thing for that dumb potential twist would be the behavior of others.

either way i doubt kagami killed kyou. if she did thered be no reason to hide kyou's death from the readers. he was definitely killed offscreen for another twist a bit later

also bet the author thinks they're brilliant for the veranda death considering it was foreshadowed in chapter 1. even if the plot is a mess from the manga getting axed, i guarantee that the veranda death was planned ages ago.

I hate when mediocre people don't stick to doing what they're the best at. It is as clear as day that author can't write anything that isn't a hentai. So why did he decide to be brave and try writing a proper manga?

Surely, at some point along the way you should question yourself, "am I really built for this? Do I really have talent?"
