This premise is really getting stretched to its limit. I like that that he likes her teeth but I'd also like if they had anything else in common or to talk about other than teeth, her teeth, their history with teeth, the trauma of having different teeth, etc.

I feel like we're only a few chapters away from an evil dentist showing up as the main antagonist.
Yeah, I'd appreciate it if his backstory was something other than:
"You know this one characteristic of yours? I am so fucking into it people called me crazy, but I am glad you're also into it tho"

Change "teeth" to "pretty feet" and you'd convince me this was Kojima or Tarantino's take on a highschool romance manga.

Are you saying that this manga that is clearly a weird showcase/argument for the author's fetish focuses to much on the author's fetish?
To be fair though, that's basically what a romance story is supposed to be. They're just the author's "hear me out". When writing a romance, you're trying to sell the reader on a relationship and the beauty/attractiveness of the characters. With more typical fetishes, they're still trying to sell you the idea, it's just not as noticeable or has a lot of effort since it's assumed you've already been won over. Probably half the reason harem romance feels so lackluster most of the time is them using common templets and not really pushing why a specific girl is so great.

There's actually a korean isekai novel that's a good example of this, Infinite Evolution Starting from Zero. When I got to the end, there was an author's note explaining how the author thought all the game mechanics were boring but the heroine was their absolute favorite part of the story, to the point where all the fighting and game mechanics were just a backdrop for the two's story. You could tell she was the author's 'ideal girl', this childhood friend who he keeps talking about her pretty feet and has a bladder problem that keeps her wetting the bed and needing diapers all the way into university (even after she overcomes it, she still pisses herself during sex of course). That said, the author actually puts the time to fill her out as a character, showing her anxieties over trying to keep up with an isekai protag boyfriend or viewing herself as disabled (because of pissing herself) and her general struggles. In the end, the author did convince me of the attractiveness of his adult diaper wearing, pissy pants, pretty feet, childhood friend heroine. Bro was just that committed.
