It's always been some combination of all involved parties' desires within the limits they, consciously or not, have set for themselves. Yuuki's unwillingness to have sex with someone who may not be completely willing acts as a brake, for example (the reason the creators are still keeping Tenka away from holding the reins). But it's often uncertain just where the actions in the reward are coming from (I'm thinking specifically of the footjob Kyouka gave a long time back; both were super skeeved out about it).

It's been easier for the girls to push responsibility onto Yuuki than to examine their own repressed desires, especially since the ones who aren't repressing them don't seem to be allowed to give rewards (Tenka obvz, but Shushu's also been left mostly on the outside looking in since deciding to be honest with her feelings).
