what comes closest in around the same era is the Nock Gun. https://en.m.wikipedia.org/wiki/Nock_gun#:~:text=The Nock gun was a,powerful recoil and eventually discontinued.

this was designed as a naval service volley gun that fired seven musket balls at once. it was not much of a success BC of the enormous kick that could either bruise or even break the shoulder of the person firing the gun. it was however popularized by writer Bernard Cornwell who featured it in his books about rifleman Richard Sharpe. rifleman Paddy Harper uses one as a personal weapon

the reference also reminds me of the Pumpkin Scissors manga where character Randel Orland uses an enormous single shot pistol which is able to take on tanks (although that was based on the Center/ Contender single shot hunting pistol)
there is also another gun called the abus gun from the Ottoman empire https://en.wikipedia.org/wiki/Abus_gun

I am guessing that Musket Girls are at the Gate waiting to give the enemy that Breaches trough the Gate, the Warm Welcome of Gunpowder and Bullets...AKA WWGB

Well the artist modeled it after a castle with them because there wouldn't be a reason for the walls to jut out otherwise. Grenades were used during sieges during this period, it would be strange not to have any.
A design can be copied without understanding the reason for that design; for example, think of how many times you've seen that same style of castle in video games and didn't have machicolations. I can guess it's going to be a lot. To an artist who's never researched castles before, the reason they jut out is because that's just what you put on a castle, it's an aesthetic like greebling is in scifi. I think it could've just been some oversight in just making the room plain and square since the mangaka is specifically detailed about the warfare stuff. And if you look at chapter 6 the walls at the gate it looks like they were drawn with empty spaces for them.
For the grenades understand that we haven't seen any present in the manga whatsoever, and this is not our world either so it's possible they just don't have them in that setting. Or it could be they didn't have any at that fort for any number of logistical or political reasons (outdated fort of lesser importance and other places need them more), or the mangaka didn't think about it, or it's not plot relevant yet and they're hiding in a box somewhere. They can certainly make improvised explosives like what the suicide bomber did in chapter 7 with kegs.
IF they have grenades then it would be a great opportunity for them.

Completely different topic, but, since he decided to introduce rifling and Minie balls, will MC be able to keep himself from introducing more advanced medical procedures, like making the doctors and nurses wash their hands and boil their tools (assuming that they don’t have basic sterilization yet)?

The Abus gun one seems to fit both visually and Krombertz description of it's role the best. At least compared to wall guns or the nock gun. SInce it's doesn't seem like a long gun nor a mulit-barrel gun.

I can definitely see how it could be used to both spook the enemy into being suppressed and breaking down the fort door since they're basically a 'portable cannon'.
