Honestly, both of those sound equally nonsense to me.

But either way, consider this.
Nekotoufu made this spin-off, where both chapters come dangerously(?) close to autogynephilia. He made chapter 76, which insists that "a penis doesn't belong in yuri". He made multiple chapters where Mahiro gets aggressively horny whenever he has his comrade, and it's even worse when that happens to Momiji. He made what could become a perfectly good "trans girl coming out" story, yet he refuses to commit to that narrative in any way that wouldn't require a lot of selective perception.
He's not your ally. He's not your enemy either, but he doesn't share your western sensibilities.
Why do you expect him to write his characters according to your specific values, rather than by the tropes embraced in anime?

Euphoria boners are a thing, are well documented, and experienced by basically every trans person, amab or afab.

Also you forget that things are written by people, and trans people are a well documented phenomenon prevalent throughout all societies, even having people in the Bible that have a similar role and position that fit with our modern idea of trans people.
