Alas, given how these sorts of stories are structured, that won't be resolved until a big "he confronts her about his trauma" moment to get everything out in the open.

Something will happen to trigger his PTSD—maybe Izumi and the brother will run into each other at the mall, and then Tohno will spot them really hitting it off (probably by talking about him, but he won't know that); perhaps they'll be trying to plan a surprise birthday party for Tohno and Izumi will innocently ask for the brother's contact information so they can rope him into it; could be anything, really—and it'll explode for a couple chapters while he works through it all, has his emotional confrontation, and they resolve it.

I'm placing bets that he has to get the obligatory return cold. In his fever induced high, he mistakes yukimura for her, while she walks in to return the favor. OR he overreacts when he comes out of his room with fever induced high and sees her talking with his brother because she brought a reciprocate care package.

Tis one of my favourite clichés in high school rom-coms. All the tsundere attitudes melt away when genuine concern forces a crack of honesty that they can't walk back from. This and the "Princess Carry" to the school infirmary, showing the ML push past any embarrassment to take care of the FL with no regard for being ribbed or mocked by classmates.

"I guess I can go home after she falls asleep" or whatever that line was, has a pretty high chance to be a flag.
If not, then we still have 'parent/siblings come home as char is leaving', 'other char gets sick and the positions swap', 'embarrassed avoidance', and 'friends find out and tease them (sometimes even leading to feeling realization) to get through. Take your pick!

(Though personally aside from whatever happens with those, they'll do some with the planned not-date too, where he plans/does something so she doesn't have to hold back on wanting stuff.)
