EDIT: Btw @Esper @kariocean I went back and looked and, unlike the other girls, Lilia is first penetrated orally. Since the key seems to be penetration, that might make a difference.

Perhaps MC just thinks something isn't quite right. The moment after finding out MC's a dude, Pinako asks him to fuck her specifically so she can see and talk to Noa. The same girl who was adamant that Noa was the monster before MC revealed that not to be the case. That same Pinako now wants to see and talk to Noa. MC is kinda a smart guy, so given Pinako's reason and her subsequent attitude about actually doing the deed, it probably raised a red flag for him. So how he's treating her is calculated, if I had to guess. He wants some answers to her motivations.
