Also, they're definitely in a game world. That backstab shouldn't have killed the guy normally, but if you have:

• Backstab: Deal 300% damage to <Unaware> targets or targets with <Unbalanced> status.
• Sly Banter: Chance to inflict <Unbalanced>. Guaranteed if used to initiate combat.
• Dagger Specialization: Increase damage dealt by daggers, scaling with <Dexterity>.
• Endseeker: Damage greatly increased against targets with low health.

... then it makes sense.

Stab in the spine, damage the nerve cord, then kick the now-paralyzed man into the river where he drowns.

And with enough force and knowledge of the body (neither being out of the realm of reasonable possibility, for sure), damaging the central nerve cord in the spine with a dagger isn't a big ask.
