I think the joke is, at a glance she 100% looks like a dude , but she has so much on (B thing , sarak..thing, shoulder pads) to keep her chest tight that when she laughs it sounds like "he he he" like , the sound a dog or a cat makes in heat. Try to do the noise of a cat in heat breathing and I think it makes more sense. Thats how I got understood it anyway.
