Considering how old she is, and its been like 70 years, at most she would be a Granddaughter.
I mean, from what i remember, Careena didn't have any kids, and she was in her 20's when MC last met her.
And the Old Waitress we see seems to be in her 40's or higher. There is no way she could be a higher than a Granddaughter of Careena.
