Personally speaking the world building is pretty good, and the characters have a reasonable amount of depth and reasoning to them. Many of the relationships there are things that have built over time and this incident is just a tipping point in said relationships which I think is a interesting point to focus on.
The author has said that they wanted to explore a story when the hero doesn't immediately get over their physical injuries and the effects that may have on their group.
There aren't many chapters, but that because each chapter is SUPER long and takes a lot with internal thought and worldbuilding from several characters. This means it gets wordy in places and not a lot happens in the chapter, but I think this kind of slow movement makes sense for the story.
Out of 5 I would hover around the 4.3 to 4.5 range.
