(Where’s saria’s dad jokes). That aside, i could easily imagine to hear their voices while reading this, it really feels like it’s part of the story. It’s also great to see the correct height of ifrit here when she’s always drawn as a small kid in other works.

I think this one actually have the most in-character one for all three. Most people will simplify silence "hate" as in " if I see your face I will kill you", but it is super ooc and make a character become so shallow( hate or love there is no in between). it is more intrigued to depict this as a kind of unresolved tension between Saria and silence, they reluctantly confront each other rather than not seeing face to face. Also they pretty mature according to the lore, so I expect them behave more like adults ( likenin this doujin) than fighting

On a more serious note, it really makes me wonder what kind of experiment doctor did in the past (in
Babel
). I mean, he was
a ruthless tactitian who didn't care about deployed OP prior to amnesia, but you don't call a tactician a "doctor" right? Meaning that he is indeed a doctor, that may have done some horrible experiments at that time, probably even worse than Rhine labs.
...wasn't it spelled right out in the very early story parts that the moniker DokuhtahDoctor comes from being a trained neurosurgeon or somesuch?
Not seeing how that would automagically translate to envelope-pushing mad science like (certain parts of) Rhine seems to be big on. Especially if they were a wee bit busy running a freaking war.
@Liu, @RNDM1, Neurologist, the Doctor got a degree in Neurology. Thus, the moniker doctor always fits, though not the medical one, that is Kal'tsit, but the professor/theoretical one.
It's assumed that Doctor is the head researcher for creating the PRTS (Primitive Rhodes Island Terminal Service, also often nicknamed Priestess), thus until today the Doctor is the only one capable of using all of its feature, in exchange for sanity.
Don't underestimate the PRTS, it literally can accurately deduce any route any enemies will take, gives eagle-eyes view of the whole battleground, allow instant communications between Doctor and the operators regarding when and where they must deploy and use their skills, accelerates or slow down the perception of time for Doctor, and allow Doctor to send his consciousness back to Rhodes Island virtually from anywhere to check on base and other business.
I believe PRTS is the reason why
the Doctor kinda shift role from a researcher to that of a tactician back in Babel, because the system can be used only by him. Either PRTS is the Doctor's own idea after feeling he need to contribute more directly, or the idea was pitched by someone else to help Theresa win the war as trump card.
So in conclusion:
-The Doctor does have a doctorate degree, so the codename likely come from there, and even after he changed job to tactician, his allies are already accustomed to refer to him as Doctor.
-If there is any crazy research he has done pre-amnesia, the PRTS is implied to be one.

Worth noting that in Ifrit's in-game talk lines, she cares almost none for the Doctor - they're just another coat at first, and the best it gets is that Ifrit actually acknowledges they're an individual with authority. It's always Saria and Silence first, and the person she's talking to second.
"Saria's here? Let me talk to- Silence said I can't? C'mon, what's the big deal? Bring me to her and just keep it off the record!"
"What's going on between Silence and Saria? I don't understand... Hey! You know how to get them to make up? Tell me... won't you?"
"You can do whatever you like with me, but you better well be nice to Silence. When she wants something, you go get it for her! Do that, and I'll follow your orders from this day forward!"
