In Japanese, he is saying "ah," which is similar to the English "yeah," but in this case it's just the sound that his lungs are making when she is carrying him. He is already dead. I really dislike the path that Hamita took with this, but I guess it is on brand.

Honestly, it’s my fault for expecting a good ending in a Hamita comic, this is like drinking battery acid and expecting it to taste like a smoothie.

I expected more from myself considering the other comics Hamita made, I guess I was just deluded with false hope

There's a story that I've read but can't find so it was either somewhere else or it was while I was reading here before I created an account. It's a story from magic user's POV about the hero's party returning home after confronting the Demon King, but there's a dark truth foreshadowed during the story but only revealed at the end. I won't say more because it would spoil it, in case anyone knows it and posts a link to. I hope so because I want to read it again.

He was having pain in his legs earlier and a fever, and they talked about some disease/plague going around.

Very likely he developed necrosis after his injury together with the other stuff. So yeah
