what it says in the title! i love reading things i can relate to as a chronically ill girlie myself.

i just finished the summer you were there and owieeee T_T cried at the end but it was worth it.

i generally skip over/avoid nsfw themes and gore, and if the manga does a magic-fix plotline i'll probably stop bc those are So Boring T_T

I focused it more on the "chronically ill" part, so the second one.

Disability stories are too wide and numerous to pick

Not a manga, but a nice visual novel - Katawa Shoujo.
Adult content there can be turned off in settings, but even when on there isn't many scenes of that and it's rather timid and not too graphical.
Turning off nsfw you won't miss anything that important, but I'd suggest trying to play with it turned on.

Pitching Ascendence of a Bookworm too (particularly the earlier volumes) where Myne's illness plays a (the?) major part in her story.

Also oneshots, for when you want the purest tearjerker. Can't pull any titles off the top of my head but they're definitely out there...just look for the tragedy tag and you'll find them.
