This could go one of several ways.
1. The humans double-down on this whole charade, deluding everyone else, including the twin sister, that she is the real Miko.
2. The humans keep pushing her as the real Miko until there is incontrovertible proof that she isn't. Which will take a while because, IIRC, she's actually pretty talented, just not actually the Miko. She will still attain a position of some power, but she is probably going to feel her sister somehow cheated her. And then we'll see some serious shit.
3. Same as 2, except she is declared a heretic/pariah, accused of deluding everyone, and barely escapes with her life. This may culminate in a redemption story arc.

Honestly, having also read the novel, I am worried if the manga adaptation will even get to that point.it's number 3. I don't remember the exact timing, but i think everything with the sister being the saint is already done by now. It's unfortunate that this manga adaptation has been ignoring the chapters that aren't from Lerunda's perspective; the redemption arc is one of the best parts of the WN--it gives flavor to their eventual reunion. (Many of the other "side" chapters are better than the main story line going at the time

If it's so far ahead in the web novels, then we may yet get to see the alternate perspective bits here in the manga. I wouldn't give up hope just yet.
