This is a nice way of the author to comment on how Nagatoro is different from her doujin counterpart as well as herself from the earlier chapters of the manga, how she's less sadistic and more self conscious. The author even referenced that weird kicking doujin he did in the past. Cool meta commentary.

Also the only visual differences with Senpai and the manga's president character is that the president's eyes are narrower and his hair isn't "fluffy", and yet the president is the handsome one. I don't get it.
