Using d&d rules, losing hp is generally caused by more than just something like being knocked prone. It's not like a jrpg where you quickly get thousands of hp, you have to be pretty high level to even hit 100hp. A wizard could easily never hit 100 even at the level cap of 20.

while I am interested in what constitutes 1hp, I would assume it's more along the lines of 'realistic' damage where 0 hp does not mean death, it just means you are now out of combat completely, because there is no way they they are having a mc in a world where fighting can happen regularly go down because he got poked in the shoulder
