I dunno, seems to me like it's just the usual gag and not some activist statement. I don't think the author cares about the culture of mental illness surrounding this stuff in the west. These types of characters might be appearing in manga a lot simply because the works in the isekai genre are copying each other so this joke gets replayed a lot. Portraying these types of characters as something to be laughed at is probably not a bad thing.
