Unless the patient is brought in for a panel, which can vary on what is tested (lets say 9 panel test vs 18panel test the scope changes) it really is just a matter of exposure. I treated a 70yr old once who had a new reaction to a grass they never encountered before. Just depends. It is a matter of the body mounting an immune response to something.

Japanese wasp is one fucking dangerous monster though. It's the size of literal adult finger (approx. 2") so its stinger and amount of venom injected is no joke. This thing kills people in Japan about 2x-3x more than bear each year.

You also tend to test against many things at once, so if you have one reaction, you're likely to find out most other things you're allergic to.
