There are too many penis jokes. They can be funny, in moderation, but they totally overdid it here.
Then the age gap is really concerning, especially as the girl is not emotionally mature at all. Good on the MC for not acting upon her invitation, but the way that scene played out doesn't give me a lot of hope for any future chapters.
And the whole purity thing is so incredibly weird. Why is it so important to know if a character is a virgin, and why does it have to be so often repeated?

The only thing that could save this manga is a romance between the bartender and the MC

Judging by his looks it is likely that he is around 23-24 and she 17-18 (She was talking about her future with her parents so likely she is about to graduate) so I would say they met when she was entering high school (which may have been the start of her being a shut in) and when he was entering college. I mean to be fair he always thought that she was a dude as old as he was so there isnt really anything weird about them being friends 5 years ago.

Its possible to become and engineer right out of high school if you are skilled enough. It's those corruption of minors/indecency laws that would have sent Yoshida to jail for hosting Sayu in Higehiro even if he never touched her and she denied that he did.

On the trans character, since it she "used to be" a guy instead of she "is" a guy could be read two ways. The way it reads to an older gentleman such as myself is that she had a sex change operation. The other way to read it is she considers herself female without surgery and he is being pronoun observant.
