Their mom could just be infirm like mine is. My mom got liver cirrhosis (from an autoimmune response, not drinking,) and was extremely ill for 5 years, (and sick for several more years prior to it getting that bad.) In that time, her other organs took a hit (had to have her gallbladder removed, kidneys damaged, heart and lungs weakened, etc.). She finally got a liver transplant 2 years ago and for 2 months everything looked great, but then the liver stopped healing. To this day, she has internal bleeding to some degree (more if she tried to lift anything.) She lost a severe amount of weight and had to get a PEG (stomach feeding tube.) She still has not recovered and may never recover. So I'm sorta stuck being her caretaker.
Can't say that the translator's mom is in likewise condition, but there are other life-long debilitating illness and disorders out there. So it's not that unrealistic for them to be their mom's caretaker this long, if not for their mom's whole life (unless they choose to put their mom in a care facility.)
