I don't think it was the sex itself that was the present. There was a mini storyline a while back where I think Chihiro acquired a cow print bikini/underwear set or something like that, so she probably wore it for him and that was the present. She says the present is "down below" (which I think might've meant "underneath") and then at the end he thinks about what he should cosplay as for her birthday, which would be him returning the favor.

It's a travesty that we didn't get to see it though.
