Of course I'm not arguing he should run rampant, but I would suggest he be put in some form of custody that is not outright prison.
Whether it's likely to do anything does not undermine the attempt to do so, as there will always be a possibility of reform working. Additionally, part of the issue is that ASPD is under studied because of its rarity and the fact it tends to go undiagnosed, or its patients are already imprisoned.
Also, the Utilitarian approach has it's issues, as even the most vile and disgusting people have their intrinsic rights, one of which means he has a right to both a fair trial, due process, and if he is found mentally ill and unable to distinguish right from wrong as he appears to be, able to.
I also doubt you can say that his desire to understand why people aren't helping him is so he can manipulate them per se. I think that the seven girlfriends thing shows that he genuinely wants to have people close to him but can't understand taboos or why people may not like what he did. It's manipulative, per se, as he lays out his thought process and he never suggests he's using them for his own gains, but treating them as an end in and of themselves, to paraphrase Kant since you seem to be trying to base it on some Utilitarian ethics. That's why I find his character to be so tragic, instead of deplorable like everyone else
