It seems kind of like he's doing it like he's playing an actual game with a fairly high level of abstraction. Roll a few dice and hope he'll find something out. But to be fair, a lot of the time that world behaves like a game. Which sometimes doesn't make sense to reasonable people.

You need to get bitten, or at least scratched, by the werewolf to gain the curse. MC dude would die if an angry ant bit him, let alone a werewolf. Maybe a vampire would work since you are kind of supposed to die to rise again as an undead.

Contract with a foreign power (devil/god/eldritch/witch/mage, etc) could work too, then theres the other methods, like imbibing monster parts like blood/flesh/teeth/nails.
If I remember correctly, there's a mutagenic potion one could make in dnd which requires some troll gut to make yourself into a partial troll (weak to sunlight and semi-insatiable hunger, but have higher metabolism/health, strength/agility/dexterity of troll alongside some physical characteristics showing up, like fangs, a slight shift in skintone to that of troll, height gains).

I think its possible to inflict harm without inflicting 1 damage. A very light scratch for example likely would not do 1 damage. A punch from a 10 year old to my face might hurt a little, but its not going to do 1 damage.

This game world also follows the rules where reaching 0 hp doesn't necessarily mean death, someone can heal you up with a potion or spell in time before you actually die.

But regardless it seems like they're going to maintain this 1 hp trole for a while. He might get an effect like being allowed to block one attack per long rest or something to help in the future, maybe.

A scratch that's severe enough to cause an infection would do at least one damage. But yeah, just hurting doesn't mean it does damage, which was shown with how they went to great lengths to explain why he did take damage when he got hit by Riverna.
