So based on the enemy stand-user and no testicles, Kira did the fusion dance with another guy. Maybe this universe's Part 4 Josuke and that Josuke became the base for our Josuke-Kira since the rod was kept. If Josuke kept Crazy Diamond, then maybe they got fused like Angelo. I guess the merging means the stands got merged as well.
