I cannot come up with a proper analogy for the west. The closest I could possibly going to the middle of a crowded mall with a bullhorn and announcing how big of a loser you are.
what he gets out of it is an utter humbling of his wife and her partner. They will go through with this humiliating act because as Chadsuki hints at, he can absolutely destroy both of them in civil filings,and anything they can do to lessen that destruction they will do.
I am going to try to condense this to the minimum possible.
In the west, Civil and Criminal cases go through the same court system. In Japan, this is not the case. The civil court for nearly everything is settled by extra-judicial settlements and mediation, as court cases are shameful for everyone involved.
Both sides' legal representatives will hammer our settlements based on, essentially, networth and damage done. Itsuki is pointing out he has taken "life long harm" - that is, he knows he has a solid case to obliterate both of them in civil settlements and keep hurting them for YEARS. And unlike in the west where if you are an adult and you have fucked up, in Japan it is normal and expected that if children can't pay, they will approach parents and other older relatives (grandparents/aunts/uncles/sometimes cousins if they are older) and demand payment and often get it because of how familial honor works over there.
So very literally "We both stripped naked and performed naked dogeza" will probably save them tens of thousands of Yen when it comes time to pay up as their lawyers can readily argue they apologized properly.
This is especially important for Rei who fixed to have Mahoro and Itsuki run a train on him/his networth. It will also matter for Rio because even if she divorces, even if she gets away from the local rumor mill, if he finds another guy she wants to marry, even if he's cool with her having cheated on her last husband its unlike he's going to be cool taking on her massive debt from the settlement.
